The purpose of the Physical Therapist – Birth to Three is to evaluate, coordinate, and provide early intervention home based services to children with developmental delays and their families referred to the Sauk County Birth to Three program for the Sauk County Department of Human Services. The Physical Therapist – Birth to Three Early Interventionist will provide services to promote sensorimotor functions through the enhancement of musculoskeletal status, neurobehavioral organization, perceptual and motor development, cardiopulmonary status, and effective environmental adaption utilizing Primary Coach Approach to Early Intervention evidence-based practice.

Essential Duties- Provides early intervention services with child and caretaker to help meet Individualized Family Services Plan (IFSP) outcomes in natural environments utilizing Primary Coach Approach early intervention evidence-based practice.
- Participates in multi-discipline formal evaluation of developmental skills to determine program eligibility. Assists in completion of evaluation report.
- Participates in team development, review, and evaluation of Individualized Family Services Plan (IFSP).
- Serves as primary early intervention coach for families and their children (ages Birth to 3) with developmental delays, primarily children with motor delays including services directed at obtaining, interpreting, and integrating information appropriate to program planning, to prevent, alleviate or compensate for movement dysfunctions and related functional problems.
- Provides advanced coaching and guidance to B3 team and families in the area of motor development and related natural environment intervention strategies. Provides joint coaching to other primary coaches and families when there are needs for additional gross motor development support and strategies.
- Completes on-going assessment of developmental progress.
- Completes Plan of Care reports and submits to primary care physician for approval.
- Consults and coordinates with Birth to Three team members and other community partners as needed to provide the best possible early intervention services.
- Demonstrates sensitivity to cultural, linguistic, social, economic, educational, individual, and role differences among families served.
- Works as a collaborative, productive, and positive team member in promoting early intervention services within a strong teaming environment.
- Completes necessary documentation and paperwork timely to meet the federal, state, and local Birth to Three standards for documentation and insurance revenue purposes.
- Assist with child find, community awareness, parent/family networking activities and educational activities for families in the community.
- Assist with state file reviews and program self-assessment process.
- Participates in weekly staff meetings.
- Other duties as assigned.
Minimum Training and EducationRequired:
Bachelor’s, Master’s, or Clinical Doctorate degree from an accredited Physical Therapy ProgramLicense:
Wisconsin Physical Therapist License
Valid driver’s licenseAny combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
